This is a short questionnaire to help Dr. Stone see what your main concerns are and the best way to get started on your healthcare journey. Make sure you click on the green submit button at the bottom of the application - this let's us know it's ready to be reviewed so you can move on to the next step in the process.
Schedule your DISCOVERY VIDEO CALL.
We review all applications within 48 business hours. Once reviewed, we'll reach out via your preferred method of contact with a link to book your discovery video call. If we have any questions before booking your discovery call, we'll let you know.
Schedule your NEW PATIENT INTAKE or VIRTUAL VISIT.
All primary care patients in the state of Arizona are required to do an in-person visit. If you're out of state or looking for specific care that doesn't require a primary care visit, you have the option of booking a virtual visit. First visits are typically at least 60 minutes so we have enough time to review your history, past labs, perform necessary exams, and determine the next steps in your plan.

What's Next?
Once you've gone through the on-boarding process and established care, Dr. Stone will lay out your care plan. This typically involves several things:

Determine if any testing needs to be done - basic blood work, specialty testing, imaging, etc.

Based on results and history, Dr. Stone will put together a comprehensive plan. This includes any referrals, lifestyle changes, supplements, medications, or other treatments. She'll review risks and benefits of any recommended treatments so you can make an informed decision about your healthcare.

You'll discuss expectations for treatment - how long it will take to see improvements and how long it will take to reach your healthcare goals.

Dr. Stone will let you know:

How often she'll be checking in with you through the patient portal

If follow-up testing is necessary, she'll discuss what needs to be retested and when.

And she'll let you know when your next phone or office visit will be.

For most cases, you will have more frequent visits initially to make sure you're getting well and staying on track. As your health improves, the length between visits is extended and you'll be moved into a maintenance plan. The ultimate goal is to get you to a place where you only need visits 1-2 times a year for prevention and monitoring. We want you to feel more in charge of your health!
